Post Care Partners was born from a moment of fear and a flash of clarity. When my sister came home from open-heart surgery at 29, our family rallied around her. But a week later, everyone had to go back to work, and she was alone.
What happens to the people who go home to an empty house?
I looked for an answer. I couldn’t find a reliable one. So I built it. We show up so no one has to recover alone.
